In accordance with 45 CFR 164.502(e)(1)(ii) and 164.308(b)(2), if applicable, Carepaths will ensure that any subcontractors that create, receive, maintain, or transmit protected health information on its behalf agree to the same restrictions, conditions, and requirements that apply to the Carepaths with respect to such information.
Information from Clinicians
When a clinician provide us with biographical information and photographs, we will use that information to create a profile of the clinician that will appear on public portions of our Site. Carepaths and its affiliates may use this information to contact you in the future to tell you about services we believe will be of interest to you. You will be able to edit this information as needed. If at any time you wish not to receive any future communications or you wish to have your name deleted from our mailing lists, please contact us as indicated below.
Carepaths may disclose PHI, as needed, to a clinician that the visitor has selected as a service provider for the purpose of facilitating the provision of healthcare services. Carepaths may also use protected health information for the proper management and administration of the site or to carry out its legal responsibilities.
Carepaths may disclose data collected if required to do so by law or in the good faith belief that such action is necessary to (i) comply with a legal obligation, (ii) protect and defend the rights or property of Carepaths, (iii) act in urgent circumstances to protect the personal safety of users of the Site or the public, or (iv) protect against legal liability.
